Learning Objectives:

  • Analyze the five Priority Pillars identified in the rural medication needs assessment and their relationship to chronic disease management in rural settings
  • Evaluate the role of social determinants of health in creating barriers to medication access and optimal use, and identify potential solutions relevant to pharmacy practice
  • Design collaborative strategies to implement or expand programs such as Clinical Pharmacist Practitioners, community pharmacy-clinic partnerships, medication synchronization, and community-based paramedicine to improve rural healthcare delivery

 Description: 

Rural communities in North Carolina face persistent challenges in accessing healthcare and managing chronic conditions. This session explores findings from a "first of its kind" comprehensive medication needs assessment involving patients, providers, pharmacists, and community stakeholders. The session will review the five “Priority Pillars” identified by the assessment—Economic Supports, Patient Engagement and Health Literacy, Delivery System Reach, Medication Management, and Community Connections—and discuss innovative, collaborative strategies to improve medication access and optimize health outcomes. Attendees will also examine the impact of social determinants of health (SDOH) in our rural communities, review effective models to address SDOH influences on medication use, and identify practical ways to implement or expand these models in their own communities.
